Unknown or unidentified medications 38% were the most common cause of ototoxicity; among the known drugs, injection gentamicin 17% and chloroquine 17% were the most common implicated drugs, followed by quinine 12.7%. All the patients presented with hearing loss. Tinnitus was seen in 83.1%, whereas 22.5% had vertigo. Jun 27, 2007 Chloroquine CQ, a 4-aminoquinoline drug, has been largely used for the treatment of rheumatoid arthritis and other connective tissue diseases. Besides the well-known retinal toxicity, its use has been suspected of be associated to ototoxicity. Ototoxicity can affect anyone receiving treatment from the medications listed above. However, the likelihood of developing ototoxicity is highly dependant on which drug is being administered, duration of use, and several other underlying factors, such as kidney function and the use of other ototoxic drugs at the same time.

The Centers for Disease Control and Prevention recommend against treatment of malaria with chloroquine alone due to more effective combinations. In areas where resistance is present, other antimalarials, such as mefloquine or atovaquone, may be used instead.

Hearing loss can be even greater with exposure to both ototoxic chemicals and noise than exposure to either noise or the ototoxic chemical alone. 1 Many ototoxic substances have a greater-than-additive e.g. synergistic effect on hearing loss with noise exposure and in particular with impulse noise. 2 Several studies have suggested that some ototoxic chemicals, such as certain solvents. One such commonly used medication for dermatologic and rheumatologic inflammatory conditions is hydroxychloroquine Plaquenil, a chloroquine derivative. It is used to treat many diseases including malaria, rheumatoid arthritis and systemic lupus erythematosus. Chloroquine is related chemically to quinine and, because of this similarity, has been suspected of being an ototoxic drug. 1 It is currently being used for the treatment of malaria, amebiasis, rheumatoid arthritis, and for discoid and systemic lupus erythematosus.
