Aug 06, 2014 Hydroxychloroquine, sold under the brand name Plaquenil Sanofi-Aventis, is an antimalarial drug that has gained widespread use in treating various autoimmune diseases, including systemic lupus erythematosus and rheumatoid arthritis. 1 By some estimates, more than 150,000 patients are on long-term therapy with this medication in America alone. 2 Retinal toxicity associated with HCQ use is relatively rare, estimated at 1 percent after five years and rising with continued therapy. 3 However. Plaquenil hydroxychloroquine is in a class of drugs called disease-modifying anti-rheumatic drugs, which are used to decrease inflammation, pain and joint damage. While today it is used to treat autoimmune conditions, such as rheumatoid arthritis and lupus, it was originally used as an anti-malaria drug. The maximum dose of Plaquenil is 6.5 mg/kg of ideal body weight per day, regardless of whether a person is overweight, underweight, or normal weight. One kilogram kg equals 2.2 pounds. Women with lupus who are OVER their ideal body weight should go by these dosing rules 5’ 7” or higher - up to 400 mg Plaquenil daily

Main Outcome Measures: SD-OCT findings suggestive of HCQ retinopathy before parafoveal ellipsoid disruption. Retrospective clinical data review by the Boston Image Reading Center.

Plaquenil hydroxychloroquine is an antimalarial medication used to treat or prevent malaria, a disease caused by parasites, which enter the body through the bite of a mosquito. Plaquenil is also used to treat symptoms of rheumatoid arthritis and discoid or systemic lupus erythematosus. SD-OCT is capable of detecting characteristic macular changes of HCQ toxicity. These typical macular abnormalities include loss of the parafoveal ellipsoid zone EZ, parafoveal thinning of the outer nuclear layer ONL and inner plexiform layer IPL, the “flying saucer” sign, and peripapillary nerve fiber layer thinning 12 – 14. The purpose of this study is to report the novel finding of early optical coherence tomography OCT abnormalities due to HCQ toxicity that may develop in the setting of normal Humphrey visual field HVF testing. Methods Data from patients with chronic HCQ exposure was obtained from seven tertiary care retina centres.
